1. Names & T-shirt Order Form – After filling in our online registration link, in order to fully complete your event registration we need to know the names of all of the students/adults you are bringing along with their preference on t-shirt sizes. For the 2011-2012 school year the deadlines for receiving this form is as follows:
- Middle School Conference Names & T-shirt Order Form – must be received via email by October 28th, 2011.
- High School Conference Names & T-shirt Form – must be received by date TBD.
After registering at our online link, please submit this form as soon as possible to the Conference Director: Tara Cowan at tara.cowan@palusa.org

2. Media Release Form – pictures will be taken onsite during the PAL® Conference. Please have your students get their parents to fill in this form along with their school permission slip. Please bring the completed Media Release forms to the conference.
